Trauma Screen

Screening for mental health is just as important as screening for other physical health issues!  Most of us go for a physical checkup once a year, but don't think twice about a mental health checkup.  Sometimes symptoms can sneak up on us gradually over time and we don't notice them until they start causing us problems.  Other times, symptoms come on rapidly and we are overwhelmed how to handle them.  We offer several mental health screens on this website to offer you a quick way to assess how you are doing in several key areas.  This is not a substitute for a full mental health assessment nor is it intended to replace mental health or medical care.  Only an independently licensed mental health or medical professional can provide a mental health diagnosis and treatment.  However, knowledge is power and these screening tools can help you become more aware of potential symptoms and alert you to seek help.

TRAUMA SCREEN:  Please review the following statements and see if any are true for you:

Have you ever experienced or witnessed one of the following adverse life events?

  • Natural Disaster

  • Fire or Explosion

  • Serious Accident

  • Exposure to Toxic Substances

  • Physical Assault

  • Sexual Assault

  • Other Unwanted or Uncomfortable Sexual Experience

  • Bullying

  • Combat or exposure to a war zone

  • Captivity

  • Life-threatening illness or injury

  • Severe Human Suffering

  • Sudden, Violent Death

  • Sudden Unexpected Death of someone close to you

  • Serious injury, harm, or death you caused to someone else

  • Other

If you answered yes to any of the above questions, please review the following statements to see if they are true for you:

  • I have repeated, disturbing memories, thoughts, or images of the stressful event

  • I have repeated, disturbing dreams or nightmares of the stressful event

  • I sometimes feel like I am reliving the stressful event

  • I get really upset when something reminds me of the stressful event

  • I have physical reactions (i.e. heart pounding, trouble breathing, sweating) when i am reminded of the stressful event

  • I avoid thinking, talking, or having feelings about the stressful event

  • I have trouble remembering important parts of the stressful event

  • I have not been as interested in activities I used to enjoy

  • I feel disconnected from others

  • I often feel emotionally numb and unable to have loving feelings for people close to me

  • I have trouble falling or staying asleep

  • I feel irritable and sometimes have angry outbursts

  • I have trouble concentrating

  • I feel super alert, watchful, and on guard

  • I often feel jumpy and am easily startled
